Thanks - I suspect it was caused by an email I sent to a customer which for some reason triggered their junk filters, although I don't know how or why that would have resulted in my account being immediately locked. Microsoft never explained the cause, so even though the account is now finally working, I will live in constant fear of it being randomly locked again at any moment...
